    The examination found: A budding tumor of 15 cm long axis extending from the anal margin, and taking all the inter-gluteal space, giving a hull crest appearance.

    A female in fant was born as the fourthchild of non consanguinous parents. The neonate had been born by vaginal delivery with poorly controlled pregnancy. The mother started taking folicacid and iodine the last month of gestation. Antenatal ultrasound didn’t show abnormalities. There was no history of congenital anomalies in any of the family members.


Tension pyothorax: Which empirical antibiotics to use

    A 49-year-old male smoker with no significant past medical history presented with shortness of breath for 2 weeks was referred to our Emergency Department (ED). He had progressive upper respiratory symptoms including mild fever and productive cough. His temperature was 37.4°C (99.3°F). His blood pressure was 116/73mmHg, Pulse rate 112 beats/minute, and oxygen saturation 96%. Significant hypoxia and respiratory distress was noted, and intubation was performed. On auscultation, he had moderate respiratory distress with scattered wheezes and rhonchi with decreased breath sounds on the right lung.

Disappearing lesion of spinal cord

    A 75-year-old man presented with acroparesthesia and unsteady gait since 2 weeks.
    His symptoms were insidious and gradually progressive in nature. Higher mental function and cranial nerves were normal. Motor system examination showed brisk reflexes, plantar flexor, absent proprioception in hands and feet and Rhombergs test positive. Patient had no history of toxin exposure and took mixed diet. Hemoglobin was 9g/dl, MCV: 110fl, vitamin B12 level 82 pg/ml. HIV was negative, intrinsic factor and anti-parietal cell antibodies were postive suggestive of pernicious anemia...


Misdiagnosed port site retained stone: A rare complication of laparoscopic cholecystectomy

    62 years old male presented with discharging sinus at the epigastric port site for 6 months which was developed two weeks following laparoscopic cholecystectomy. In last 6 months, he had undergone excision biopsy which was suggestive of epithelioid cells granuloma for which he had taken ATT for two months but got no relief. He had taken many types of empirical antibiotics prescribed by different physicians. Pus culture was done many times which revealed no growth. Transient splenium lesion

    A 6 year child presented with short duration of fever. On examination the child was drowsy but easily arousable. Investigation showed low platelets of 25,000 per microliter of blood with normal dengue serology. Diffusion-Weighted Imaging (DWI) and Apparent Diffusion Coefficient (ADC) on Magnetic Resonance Imaging (MRI) showed restricted diffusion in the splenium (Figure A and B). Sarcoidosis with multiple involvement: A clinical case image

    A 31-year-old male patient admitted to the hospital with complaints of cough and swelling on the right inguinal region. The biopsy was done on the 7×4 cm sized lymphadenopathy in the inguinal region and non-necrotizing granulomatous lymphadenopathy was reported. Bilateral hilar fullness was on chest radiography (Figure 1). Multiple hilar and mediastinal lymphadenopathy were observed through the tomography (Figure 2a-2d).
